This appears to be a valuable tool. I would be concerned that certain age groups might not be able to accurately differentiate between the different answer choices, ie. mostly true, or very true. Overall, I think that it covers a wide array of topics, and is easy to navigate. I would want to ensure that the children taking the survey clearly understand how to approach each question and what each answer choice means.
