I like the way this tool is designed as it leads to gathering a lot of rich data from students. I would wonder about the success rate of low level readers being able to complete the survey and if verbally recorded answers would change the content of information gathered. I also like the inclusion of potential answers. This would be helpful for an individual less familiar with analyzing the data as long as they use it as a guide rather than the rule.
